COVID-19 disease, or novel coronavirus, is a global pandemic of acute respiratory illness caused by the virus known as SARS-CoV-2. Most cases of COVID-19 occur in adults. Respiratory distress syndrome is an acute and diffuse lung disease that produces hypoxemia, de-creased lung compliance and increased dead space. The care of critically ill patients with COVID-19 pneumonia during the pandemic has been the most important challenge faced by the Intensive Medicine throughout its history. The treatment of pneumonia by COVID-19 has very limited evidence as it is a new disease, which has forced a permanent updating of care protocols A clinical case of a 73-year-old male patient, married, born in Quito and resident in Guayaquil, higher education, retired, religion: Catholic, laterality: right-handed. Diagnosed with Type 2 Diabetes Mellitus treated with Metformin, Empaglifozin, does not report allergies, surgical cleaning intervention for left diabetic foot. He states that he is a former smoker and social drinker, immunized against Covid-19: Astrazeneca x1 whose reason for consultation is Dyspnea, Tachypnea. Even when the relevant protocols for the clinical picture developed have been followed, the treated AKI cannot be overcome by the patient, which confirms the studies of being a picture of difficult prognosis. The effects of the new coronavirus have been exposed, but as a medical community we are still far from imposing a single guide for the management of the disease, even more so when the long-term effects of SARS-COv-2 in recovered patients are being known. As of the date of this investigation, certain elements are taken into account that can already be considered important to avoid greater severity than we have survived and lies in maintaining social distancing, complying with vaccination schedules and using face masks

La mayoría de los casos de COVID-19 ocurren en adultos. El síndrome de Distrés respiratorio es una enfermedad pulmonar aguda y difusa que produce hipoxemia, disminución de la compliance pulmonar y aumento del espacio muerto La atención a los pacientes críticos con neumonía por COVID-19 durante la pandemia ha sido el reto más importante afrontado por la Medicina Intensiva en toda su historia. El tratamiento de la neumonía por COVID-19 tiene una evidencia muy limitada al tratarse de una enfermedad nueva, lo que ha obligado a una permanente actualización de los protocolos asistenciales Se presenta un caso clínico de paciente masculino 73 años de edad, casado, nacido en Quito y residente en guayaquil, instrucción superior, jubilado, religión: católica, lateralidad: diestra. Diagnosticado con Diabetes mellitus tipo 2 con tratamiento de Metformina, Empaglifozina, no refiere alergias, intervención de limpieza quirúrgica por pie diabético izquierdo. Manifiesta ser ex fumador y bebedor social, Inmunizado para la Covid-19: Astrazeneca x1 cuyo motivo de consulta es Disnea, Taquipnea. Aun cuando se han seguido los protocolos pertinentes para el cuadro clínico desarrollado, el DRA abordado no puede ser superado por el paciente lo que confirma los estudios de ser un cuadro de difícil pronostico. Los efectos del nuevo coronavirus se han expuesto, pero estamos aún lejos, como comunidad médica imponer una guía única para el manejo de la enfermedad pues, más aún cuando se están conociendo los efectos a largo plazo de SARS-COv-2 en pacientes recuperados.
