Summary: COVID-19 disease, or novel coronavirus, is a global pandemic of acute respiratory illness caused by the virus known as SARS-CoV-2. Most cases of COVID-19 occur in adults. Respiratory distress syndrome is an acute and diffuse lung disease that produces hypoxemia, de-creased lung compliance and increased dead space. The care of critically ill patients with COVID-19 pneumonia during the pandemic has been the most important challenge faced by the Intensive Medicine throughout its history. The treatment of pneumonia by COVID-19 has very limited evidence as it is a new disease, which has forced a permanent updating of care protocols A clinical case of a 73-year-old male patient, married, born in Quito and resident in Guayaquil, higher education, retired, religion: Catholic, laterality: right-handed. Diagnosed with Type 2 Diabetes Mellitus treated with Metformin, Empaglifozin, does not report allergies, surgical cleaning intervention for left diabetic foot. He states that he is a former smoker and social drinker, immunized against Covid-19: Astrazeneca x1 whose reason for consultation is Dyspnea, Tachypnea. Even when the relevant protocols for the clinical picture developed have been followed, the treated AKI cannot be overcome by the patient, which confirms the studies of being a picture of difficult prognosis. The effects of the new coronavirus have been exposed, but as a medical community we are still far from imposing a single guide for the management of the disease, even more so when the long-term effects of SARS-COv-2 in recovered patients are being known. As of the date of this investigation, certain elements are taken into account that can already be considered important to avoid greater severity than we have survived and lies in maintaining social distancing, complying with vaccination schedules and using face masks
