Open fractures are injuries in which the fracture and its hematoma communicate with the environment through a traumatic defect in the surrounding soft tissues and the overlying skin. These injuries are particularly susceptible to both bone and soft tissue infections, and early management strategies should aim to minimize the risk of such infections. Over the years, an appropriate treatment for open fractures has been sought, achieving significant advances; however, today the complications that these entail are still a health inconvenience. By virtue of which, the main objective of this research is to summarize the clinical and surgical management of open fractures. The research was developed through a bibliographical documentary type methodology under the review modality. From the review it was found that adequate and early antibiotic therapy minimizes the risk of infection and also improves the prognosis of the injury. As for radical surgical debridement, it also reduces the risk of infection. Properly addressing factors such as quality surgical cleaning, adequate coverage of the wound, as well as definitive fixation of the fracture, favorably affects the reduction of days of hospital stay, as well as the complications of the fracture. Finally, it is important to highlight that each open fracture is different, in view of which the generalities of clinical management can be followed, however, the treatment must be adapted to each patient and each type of fracture in particular.

Estas lesiones son particularmente susceptibles a las infecciones tanto de los huesos como de los tejidos blandos, y las estrategias de manejo temprano deben apuntar a minimizar el riesgo de tales infecciones. En el transcurso de los años se ha buscado un tratamiento apropiado para las fracturas expuestas logrando avances significativos; sin embargo, hoy en día las complicaciones que estas acarrean aun constituyen un inconveniente para la salud. En virtud de lo cual, el ob-jetivo principal de la presente investigación es compendiar el manejo clínico y quirúrgico de las fracturas expuestas. La investigación se desarrolló mediante una metodología de tipo documental bibliográfica bajo la modalidad de revisión. De la revisión se encontró que la antibioticoterapia adecuada y precoz minimiza el riesgo de infección y también mejora el pronóstico de la lesión. En cuanto al desbrida-miento quirúrgico radical disminuye igualmente el riesgo de infección. Abordar debidamente factores tales como una limpieza quirúrgica de calidad, cobertura adecuada de la herida, así como la fijación definitiva de la fractura, incide de manera favorable en la disminución de los días de estancia hospitalaria, así como de las complicaciones propias de la fractura.
