Estabilización y reanimación neonatal. Actualización sobre las principales guías internacionales

Neonatal asphyxia remains a major mortality and morbidity problem worldwide. In this case, neonatal resuscitation not only prevents the death of the newborn, but also prevents secondary neurological sequelae. Current policies, plans, programs, standards, guidelines and protocols are an important step in guaranteeing the right to quality care. The purpose of this article is to present a summary of the aspects that have been updated in the main international guidelines such as the recommendations of the International Liaison Committee on Resuscitation (ILCOR), the guidelines of the American Heart Association (AHA) 2020, the European Resuscitation Council (ERC) and the Spanish Guide for Neonatal Stabilization and Resuscitation 2021 of the Neonatal Resuscitation Group of the Spanish Society of Neo-natology (GRN-SENeo). The research was carried out under a bibliographic documentary type methodology, under the review modality. The main updates of these guidelines taken for the present investigation were the following: anticipation of the need for resuscitation, informative session - Debriefing, management of the umbilical cord, management of temperature in newborns / thermoregulation, positive pressure ventilation, ethical aspects : humanization and completion of resuscitation and limits of feasibility. It is important to disseminate these updates in order to minimize morbidity and mortality rates due to neonatal asphyxia.

En este caso, la reanimación neonatal no solo evita la muerte del recién nacido, sino también evitar las secuelas neurológicas secundarias. Las políticas, planes, pro-gramas, normas, guías y protocolos actuales son un paso importante para garantizar el derecho a una atención de calidad. La finalidad de este artículo es presentar una síntesis de los aspectos que se han actualizado en las principales guías internacionales tales como las recomendaciones del Comité Internacional de Enlace sobre Reanimación (ILCOR), las guías de la Asociación Americana del Corazón (AHA) 2020, el Consejo Europeo de Resucitación (ERC) y la Guía española de estabilización y reanimación neonatal 2021 del Grupo de Reanimación Neonatal de la Sociedad Española de Neonatología (GRN-SENeo). La investigación se realizó bajo una metodología de tipo documental bibliográfica, bajo la modalidad de revisión. Las principales actualizaciones de estas guías tomadas para la presente investigación fueron las siguientes: anticipación a la necesidad de reanimación, sesión informativa – Debriefing, manejo del cordón umbilical, manejo de la temperatura en los recién nacidos/ termorregulación, ventilación con presión positiva, aspectos éticos: humanización y finalización de la reanimación y límites de la viabilidad.
